The role of Community Clinical Nurse Specialist in Palliative Care at South Warwickshire University NHS Foundation Trust involves working within an established team to deliver high-quality specialist palliative care in the community. The position is based at Camp Hill Primary Care Centre and requires close collaboration with various healthcare providers, including local hospices and hospitals. The post holder will manage a clinical caseload, utilizing advanced nursing practices to assess, plan, and evaluate care for patients with complex needs, while also advocating for patients and facilitating shared decision-making about their treatment options.

In addition to clinical responsibilities, the role encompasses leadership and education, with the expectation to supervise junior staff, contribute to service development, and enhance the knowledge and skills of colleagues through training initiatives.
