                                                    Community Support Worker - Ross District Nursing Team with Wye Valley NHS Trust in Ross On Wye

                                                     We are a community service providing care and support for patients in their own homes preventing hospital admission and facilitating early hospital discharge.   We work closely with primary and secondary care taking referrals from both hospitals and GPs.   We have a good rapport with our patients and get excellent feedback so job satisfaction is high. We take pride in enabling patients to stay at home when acutely unwell.   Our main priority for this job role is to find people with the right values, behaviours and attitudes that match our own.
