The role of Community Support Worker with the Ross District Nursing Team at Wye Valley NHS Trust involves providing care and support to patients in their homes to prevent hospital admissions and support early discharges. The ideal candidate will share the team’s core values, including a commitment to quality care, mutual respect, and teamwork. While prior experience is beneficial, the organisation is willing to provide necessary training for the right individual who prioritizes excellent patient support.

In this position, you will assist band 5 Community Staff Nurses by monitoring patient observations, obtaining blood samples, and providing wound care and education, among other responsibilities. The team prides itself on the positive relationships built with patients and aims for high job satisfaction through its commitment to quality care and teamwork.

We are a community service providing care and support for patients in their own homes preventing hospital admission and facilitating early hospital discharge.

We work closely with primary and secondary care taking referrals from both hospitals and GPs.

We have a good rapport with our patients and get excellent feedback so job satisfaction is high. We take pride in enabling patients to stay at home when acutely unwell.
