Band 3: £24,071 to £25,674 a year PRO RATA FOR PART TIME HOURS  PERMANENT 
The team consists of an extensive MDT which includes nurse practitioners, occupational therapists, physiotherapists, health care assistants, therapy assistant practitioners and administrative staff.

To support the Recovery at Home clinical staff service to provide care to people in their own homes to prevent admission/ readmission to hospital over a 24-hour period.


To work with the team undertaking roles set out in the patient management care plan.


To assist the clinical team in undertaking clinical observations to support diagnosis and treatment.


To perform diagnostic tests


To schedule own calls as directed to meet needs of the service.


Maintain accurate written and electronic information systems, supporting data input onto IT systems.


On occasion there may be a requirement to support administration colleagues with telephone calls and transferring information onto the designated clinical system

An exciting opportunity has arisen for an enthusiastic, well-motivated and flexible Band 3 Health Care Support Worker to join our Urgent Community Response and Virtual Ward Services across South Tyneside

This role is based with the Recovery at Home team based at Clarendon.

The team consists of an extensive MDT which includes nurse practitioners, occupational therapists, physiotherapists, health care assistants, therapy assistant practitioners and administrative staff. The team also works closely alongside partner agencies including those from the local and voluntary sector.

This role would involve supporting people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short-term support for their physical and functional health needs to remain at home.

The service covers 24 hours, 7 days per week, it is fast paced, and you would have the opportunity to see a wide range of patients who present with a variety of conditions.

The service is committed to providing a sound environment for education and training to support the successful candidate within this role.
