The community teams  are the strong core element of the service that focuses on longer term case management and improved outcomes for adults with issues around all substances. It has a strong key working and care plan approach and includes supporting specialist prescribed interventions and harm reduction from our prescribing team, focusing on supporting people to lead fulfilling lives within their local communities.   About us  To demonstrate expert and effective practice in complex situations, assessing and managing both new and existing service users. To manage a caseload in line with the service user profile.
