The Chippenham Community Team is seeking a full-time Band 5 Community Occupational Therapist to provide complex interventions for patients in their homes and other community settings. The role involves working with a multi-disciplinary team at Chippenham Community Hospital, focusing on rehabilitation, long-term condition management, and managing a varied clinical caseload. Occupational Therapists will conduct assessments, develop individualized treatment programs, and order necessary equipment to support patients in maintaining their independence at home. The hours of work are between 08:00 and 18:00, including some weekends.

Candidates should be registered with the Health Professional Council and hold a relevant qualification in Occupational Therapy. They will be responsible for care planning, patient assessments, and ensuring that patient and carer involvement is central to the care process.
