The Cornwall Partnership NHS Foundation Trust is seeking a motivated Lead Health Care Assistant for their Community Psychiatric Rehabilitation Service at Bodmin Hospital. This role involves working within a multi-disciplinary team to support individuals recovering from complex mental health issues throughout Cornwall. The Lead Health Care Assistant will be responsible for managing a caseload of up to 10 service users, providing both practical and emotional support as delineated in individual care plans. Additionally, the position entails supervising junior staff, allocating tasks within the clinical support team, and facilitating effective communication among service users, their families, and other health care professionals.

The ideal candidate should demonstrate strong communication skills, effective time management, and the ability to build therapeutic and professional relationships with service users and other agencies. The role requires fostering collaboration between community resources and health care services to ensure seamless support for individuals as they strive for independence outside the hospital setting. Key responsibilities also include participating in the development and review of the rehabilitation service, triaging calls, and maintaining good working relationships with various stakeholders.
