Key responsibilities include providing direct patient care by assessing medication needs, implementing changes due to regulatory alerts, and monitoring adherence to local and national guidelines. The candidate is expected to conduct audits to evaluate compliance with prescribing guidelines and safeguard policies, ensuring the welfare of all patients, including vulnerable populations. The position demands a commitment to maintaining high standards of clinical practice and collaborating with the healthcare team to optimize treatment outcomes.
The position requires a qualified pharmacist with an independent prescribing qualification, who will operate within their professional limits as a member of a multidisciplinary team that includes GPs, ANPs, Physician Associates, Paramedics, and First Contact Physiotherapists.
The successful candidate will collaborate closely with the GP and other practice clinical and non clinical staff, playing a key role in supporting GPs and the broader primary care team to provide safe and effective care. They will provide medication-related patient services, including direct patient care. As a leader, the candidate will guide and support the practice team to ensure safe and effective prescribing, while maintaining and enhancing the quality of patient care within the practice.

- Patient-facing long-term condition clinics:
Consult with patients who have one or more medical conditions that require medication optimisation (e.g., COPD, asthma, hypertension).
Review the ongoing necessity of each medication, assess monitoring needs, and provide support to patients in managing their medications, ensuring they achieve the best outcomes from their treatments (i.e., medication optimisation).
Medicines Safety:
Implement changes to medications in response to MHRA alerts, product withdrawals, and other local and national guidelines.

Implementation of Local and National Guidelines and Formulary Recommendations:
- Monitor practice prescribing in accordance with the local formulary and recommend to GPs which medications should be prescribed by hospital doctors (red drugs) or be subject to shared care (amber drugs).
- Audit the practices compliance with NICE technology assessment guidance.
Safeguarding Children, Young People, and Vulnerable Adults:
- Comply with safeguarding policies and support the commitment to promoting welfare.

- Comply with all Hounslow Family Practice policies and stay informed about any changes.
