Nottingham City GP Alliance is looking for an Advanced Clinical Practitioner (ACP) to join their GP surgery teams across Nottingham. The ACP will play a vital role in providing high-quality patient care for a variety of acute and chronic conditions, working both independently and collaboratively with other healthcare professionals. Responsibilities include assessing, diagnosing, and treating patients, prescribing medication, managing treatment plans, and providing health education and disease prevention support.

Candidates should hold a Master's degree in Advanced Practice or have equivalent experience, along with independent prescriber status and extensive clinical experience in primary care or similar settings. The ACP will contribute to innovative, team-based care models aimed at addressing the needs of Nottingham's diverse populations, particularly in areas with significant health challenges and complex needs.
