The Liverpool University Hospitals NHS Foundation Trust is seeking an Advanced Respiratory Practitioner for the Liverpool Community Respiratory Team and the Acute Respiratory Infections Virtual Ward. This role focuses on providing hospital admission avoidance and facilitating early discharge for patients with various respiratory conditions such as COPD and pneumonia, through assessments from patients' homes and acute hospital locations. The practitioner will engage in high-level clinical decision-making, develop individualized care plans, and collaborate with a multidisciplinary team. Responsibilities include delivering specialist training, maintaining high-quality service standards, and fostering partnerships with other healthcare services to enhance patient care.

The position requires strong theoretical and practical knowledge in respiratory care, as well as a commitment to delivering anticipatory and responsive care for both planned and unplanned needs. This role involves significant mobility, working across multiple sites with a schedule covering weekends. The Advanced Respiratory Practitioner will also mentor staff in caseload management while ensuring effective implementation and evaluation of care plans for patients with chronic respiratory conditions.
