Band Specialty Doctor: £59,175 to £95,400 a year per annum plus London Weighting  PERMANENT

The successful applicants will be based in one of the integrated community mental health teams in Hammersmith and Fulham as part of the community and Recovery Mental Health Services service line. The service will comprise a fully integrated mental health team with a visible presence in the local Primary Care Network. There will be a tiered or stepped care model within the MINT team where a more primary care ethos and model of delivery will be used for the majority of patients but those who have more complex needs will have more enhanced support from an allocated clinician as well as the multidisciplinary team. The post will therefore involve consultation/liaison with the MDT as well as GPs, primary care services and other mental health teams. Alongside this there will be some direct consultation with patients particularly around complex cases and crisis prevention. Key working relationships will be with the GPs, Crisis Team and Early Intervention in Psychosis Service.
