We currently have vacancies for Community Staff Nurses in our Cannock Villages Community Nursing team based at Norton Canes Medical Centre.
We are looking for a Qualified Nurse to join our busy, supportive & friendly team.Our Community Nursing teams assess, plan and deliver nursing care to patients within their own homes, in line with evidence based practice. Many people would be unable to live at home without the support they receive from the community nursing teams and community services are admitting patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff.

A position in our community nursing teams could be for you if:
You have a commitment to providing high standards of holistic care to our patients. You are able to work effectively on you own, and work well within a team, demonstrating a wide-range of clinical skills.
You will be competent, or willing to gain competency in skills such as palliative & end of life care, long term condition management, diabetes care, medication administration, complex wound care and bladder & bowel management.
Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.
You will receive support into the role through induction, training and beyond. There will be opportunities for personal development and career progression if desired.
What we ask from you:-
Role
Working in collaboration with the muti-disciplinary team and colleagues to:-
-Promote independence and support patient centred care planning.-Provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration).-Work autonomously, in line with your scope of practice, competence and policies.-Handover patient information to the wider team, document patient information effectively.-Escalation of patients needs as appropriate.
Qualifications and training
Essential-Degree in nursing or equivalent, including NMC registration. Practice supervisor training or a commitment to work towards.Desirable- Completed a placement within a community service. Genuine desire to work in a community nursing team.
You must hold a full UK driving license and have the ability to travel around the geographical patch.
