This is a rare opportunity to help deliver our vision for an integrated system-wide approach to Frailty and Acute Medicine with one of Cheshire & Merseyside's District General Hospital.

East Cheshire NHS Trust strives to be one of the best performing frailty services, the service provides diagnosis, treatment, and facilitates early discharge of frail patients and provides interface with frailty virtual ward. The virtual ward has a dedicated team supporting the Frailty team in both the Emergency Department and across all the acute medical wards.

The Frailty team is a multi-disciplinary team based in the Emergency Department and the trust is currently looking to create and run an Acute Frailty Assessment unit delivering same day emergency care for patients with frailty as part of the frailty approach across the hospital. The successful candidate will support the implementation of the Frailty SDEC alongside the clinical and operational teams. This is a new initiative for East Cheshire Hospital to support the aging population of Eastern Cheshire.


Hours per week: 10 PA's

  • Provide a high quality, safe and compassionate care to the acutely ill medical patients admitted to our hospital and provide them with the best possible experience during their admission.
  • Active participation in Trust Clinical Governance activities, including audit, mortality review and appraisal process. The appointee will ensure data collection and processing undertaken is consistently accurate, timely and complies with the Trust Data Quality Policy and Information Governance procedures.
  • Teaching, training and supervision of doctors, nurses and students in order to promote a positive and supportive atmosphere.
  • Responsible for keeping a record of Continuous Professional Development (CPD), including Trust Mandatory training process.
  • Will be responsible for engaging in regular appraisal processes for revalidation
  • The provision of a first-class clinical service.
  • Effective leadership to all staff engaged in the specialty.
  • Sustaining and developing teaching and research.
  • Undertaking all work in accordance with the Trust's procedures and operating policies
  • Conducting clinical practice in accordance with contractual requirements and within the parameters of the Trust's service plan
  • Maintaining the confidence of business plans and development strategies formulated for the speciality, the Care Group of the Trus
