This post is for a Trainee Advanced Clinical Practitioner working in Urgent Community Response and the Frailty Hospital at Home across Mid Hampshire. The post holder will be a trainee, working under supervision, to develop a high degree of autonomy and complex decision making. This will become underpinned by a MSc level award, completed during the training programme that encompasses the four pillars of clinical practice; leadership and management, education and research with demonstration of core capabilities and area specific clinical competence. They will manage, under supervision, clinical care in partnership with individuals, families and carers, which will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.
Assessment, diagnostics and treatment of acutely unwell people who are at risk of hospital at admission
Review of patients and their progress, with relevant adjustment of treatment plans
Application of the four pillars of advanced clinical practice to the role
The post holder will received supervision and development from qualified advanced clinical practitioners and consultant practitioners
80% clinical working and 20% across the remaining 3 pillars
