The Bank Band 6 Community Nurse role at Central London Community Health Trust in Wembley involves delivering high-quality nursing care to patients in their homes and various community settings. The position requires the nurse to provide rapid response, rehabilitation, and post-acute care, operating within a shift pattern from 8 AM to 10 PM, including weekends and bank holidays. Responsibilities include holistically assessing patients' needs, managing a caseload in collaboration with the Team Leader, prioritizing referrals, and developing comprehensive care plans tailored to individual patients, all while focusing on service priorities like avoiding admissions and ensuring timely discharges.

Applicants must be NMC registered nurses with at least six months of acute experience, as the role demands the ability to provide clinical care to patients with complex health needs. Strong verbal and written communication skills, as well as effective team collaboration and interpersonal abilities, are essential for success in this independent nursing position. The shifts are long, with 12.5 hours and one-hour breaks, and the role is offered on a zero-hours basis, meaning it does not attract visa sponsorship.
