The Care Plus Group is seeking an Assistant Practitioner for their CPG Rehab, Re-enablement, and Recovery unit, a facility that aids individuals in regaining independence following illness or significant events. The role involves working within a multi-disciplinary team, collaborating with healthcare professionals to support patients through their recovery journey.

Key Responsibilities:

Coordinate patient care shifts over 24 hours, addressing physical, social, and wellbeing needs.
Collaborate with a multidisciplinary team, community nurses, GPs, and case managers to implement tailored support plans.
Participate in the admissions and discharge processes, supervise staff, and assist with clinical and social care tasks along with daily operations of the unit.

Requirements:

Foundation Degree or Higher Education Certificate in Integrated Care or Level 3 in health/social care (with a commitment to achieve level 4).
At least 2 years of experience in a health or social care setting.
Proficiency in Microsoft Office (Word, Excel, Outlook, Access).
Availability to work a flexible rota, including evenings, weekends, and bank holidays.

Salary: £24,645 - £27,202 pro-rata per annum.

Location: Community Inpatient Unit, Peterhouse Road, Grimsby.

CPG Rehab, Re-enablement and Recovery unit is a bed-based facility supporting people to recover and regain their independence following a period of illness, trauma, surgery, or significant event. It supports hospital discharges, prevents admission to hospital or long-term care and promotes independent living. Its a multi-disciplinary service with Nurses, Physiotherapists, Occupational Therapist, Social Workers, and care staff.

We provide a range of therapies and support to assist people in their recovery and regain their independence, enabling them to return to their own homes safely and with any additional support in place.

Job Title: Assistant Practitioner

Base: Community Inpatient Unit, Peterhouse Road, Grimsby, DN34 5UX

Hours: Full-Time and Part-Time hours available.

Salary Range 4: £24,645 - £27,202 pro-rata per annum

You will support the co-ordination of shifts over a 24hr period ensuring that patients are supported throughout their journey with their physical, social and wellbeing needs. You will work in liaison with a wider unit based MDT and a range of other health care professionals, including community nurses, GPs, and case managers to implement bespoke plans of support.

Other duties include input into the admissions and discharge of service users, staff support and supervision, clinical and social care duties and practical duties around the day to day running of the service.

It is essential that you hold a Foundation Degree or Higher Education Certificate (level 4) in Integrated Care or other appropriate topic OR a Level 3 in health or social care with willingness to work towards level 4 on appointment. You will have experience and competence in Microsoft Office applications: Word, Excel, Outlook and Access as well as a minimum of 2 years proven experience of working in a health and/or social care environment.

You must be able to work a rota pattern including evenings, weekends and bank holidays.
