The Complex Case Worker position with North Staffordshire Combined Healthcare Trust is a 12-month fixed-term role primarily designed for qualified nurses specializing in learning disabilities or mental health. The role involves supporting the Transforming Care Partnership (TCP) program by facilitating the delivery of safe and effective care services for individuals with learning disabilities and autism. Responsibilities include coordinating care plans, preventing hospital admissions, and supporting discharge from facilities, while being the key contact for health and social care professionals, service users, and their families.

In this position, the post holder will be part of the Learning Disability Transforming Care Team, reporting to the Service Manager and contributing to the overall management and coordination of the TCP. The role emphasises adherence to Trust policies, performance expectations, effective resource management, and building strong relationships for efficient service delivery. Additionally, the Complex Case Worker will be involved in the ongoing development and training of team members, maintain proper documentation, and ensure compliance with confidentiality standards.
