St Michael’s Hospice in Hereford is seeking a qualified and experienced Social Worker to join their Supportive Care team. The role involves providing direct psychosocial support to patients, families, and carers within the Inpatient Unit and Day Services, addressing various issues such as cultural, spiritual, psychological, and financial needs. Key responsibilities include assessment and planning for service user needs, managing a diverse caseload, and collaborating with the multidisciplinary team to address safeguarding concerns in palliative care.

The ideal candidate will possess strong communication and interpersonal skills, be highly motivated, and able to work autonomously while maintaining confidentiality. This position also involves engaging with external agencies for support provision and participating in the development of services through grant applications and community resource optimisation. The role aims to enhance the social and emotional well-being of those affected by illness and bereavement, fostering a collaborative, rehabilitative approach to care.
