The Urgent Community Response team are looking to expand the team of Integrated Support Workers across East Sussex with opportunities to be based in either Eastbourne or Hastings.
East Sussex Healthcare NHS Trust is an integrated Healthcare Trust combining acute and community services. The Urgent Community Response team supports patients within their own homes across the UCR, HomeFirst and Virtual Ward pathways. The ISW role is an opportunity to provide outstanding care and support to patients in their own homes. The role involves working alongside Senior ISWs, Nurses and Therapists to optimise patient outcomes. ISWs provide personal care, meds support and support with eating and drinking as well as working towards further competencies to further support the Multidisciplinary team.
The Urgent Community Response team work alongside the integrated night service who provide cover overnight
