Purbeck Health Centre in Milton Keynes has a fantastic opportunity for an experienced practice nurse to join their well established team. This position is offered on a full or part-time basis with hours between 30-37.5 per week. The ideal candidate will provide high-quality patient care and support in a medical practice setting, contributing to the delivery of quality healthcare services.

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Running asthma, COPD and diabetes clinics and reviews.
