CASELOAD MENTAL HEALTH MULTI-DISCIPLINARY
The Community Mental Health Team at Betsi Cadwaladr University Health Board is looking for a skilled and motivated Community Mental Health Nurse to join their established multi-disciplinary team in Denbigh. The role involves care coordination and case management for a designated caseload of clients with severe and enduring mental health issues, ensuring adherence to Care and Treatment Plans in line with the Mental Health (Wales) Measure 2010. The nurse will be responsible for conducting comprehensive assessments, managing clinical risks, and delivering crisis interventions, while also supporting the administration of medication and physical health monitoring.

Successful candidates will work collaboratively with service users, families, and partner agencies to provide high-quality care and support. While the ability to speak Welsh is desirable, applicants who speak either Welsh or English are welcomed. The position aims to enhance the care provided to individuals facing complex mental health challenges within the community.
