EMPATHY FAMILY MULTI-DISCIPLINARY NHS SOCIAL WORK
The Community Practitioner position within the Older Persons Community Treatment Team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust is aimed at a motivated individual with experience in working with older adults. This role, based at the Centre For Aging and Vitality in Newcastle, involves assessing and providing treatment to clients in various settings, including community care homes and their own residences. The practitioner will act as a Care Coordinator, collaborating with a multidisciplinary team and other agencies to develop and implement recovery-focused care plans for service users with complex needs.

Key responsibilities include managing casework, providing clinical advice, delivering evidence-based interventions, and ensuring the involvement of carers and families in care planning. The candidate must possess strong interpersonal and communication skills, be comfortable dealing with risk management, and work flexibly to meet the needs of service users. The role requires working Monday to Friday, with potential for extended hours, and a commitment to delivering compassionate, client-centered care.
