CASELOAD END OF LIFE FAMILY HCSW NHS NMC ON-CALL WEEKEND
United Lincolnshire Hospitals NHS Trust is seeking experienced children's nurses for the expanding Community Children's Nursing (CCN) Service in Boston. This Band 6 role involves working as part of a multidisciplinary team to provide comprehensive care to children transitioning from hospital to home. Responsibilities include managing a caseload of children with acute healthcare needs, coordinating services to facilitate early discharges, and participating in an evening on-call system. Candidates should be NMC Registered Nurses (child) with community caseload management experience preferred, and a commitment to empowering families for self-care is essential.

The role requires excellent communication skills, the ability to work autonomously and collaboratively, and the willingness to travel throughout Lincolnshire. Working hours are Monday to Friday, 9am to 5pm, with the need for flexibility for school visits and participation in an on-call service.
