We are looking to recruit enthusiastic Health Care Assistant who have a genuine interested in providing safe and effective care for patients at home. The successful candidate must have demonstratable experience, a wide range of clinical skills and will be confident to work without direct supervision. Community nursing is evolving at a fast pace, and you must be able to engage positively with service improvements and change. We invite Heath Care Assistants with previous community experience to apply, but this is not essential as successful candidates will be offered training and support.
To assist the Community Nursing Team in the provision of nursing care and support to patients and families in the community setting.
Duties and Responsibilities:
To work as part of an integrated Community Nursing Service in providing specific care to a patient in the community setting as written in the care plan.
To provide personal care and support to palliative patients in their own homes.
To work alone and unsupervised and the ability to work autonomously.
To be responsible for the ordering of clinical supplies and stationery for the Community Nursing Team.
To work in accordance with Trust Policies and Procedures.
To maintain high standards of personal hygiene adhering to Health and Safety Guidelines and Infection Control Guidelines within the trust.
To respect confidential information during the course of your work and refrain from discussing such information without the consent of the patient/client or a person entitled to act on his/her behalf, except where disclosure is required by law or by the Order of a Court or is necessary in the public interest.
Clinical
To monitor skin integrity of patients at risk of pressure sores formation and provide basic preventative information.
To provide full personal care to patients in the terminal phase of life.
To provide health screening for household patients e.g. Monitoring of blood glucose levels, phlebotomy, blood pressure recording and urine testing.
To provide clinical intervention for care of patients as directed by Registered Nurses, following training and assessment of competencies e.g. administration of insulin, basic wound care and bladder irrigation.
To check equipment on a regular basis and ensure it is safe to use.
To attend mandatory training in accordance with the Trust Policy.
To identify areas for development within a Personal Development Plan in collaboration with District Nurse coordinator/District Nursing Sister.
To report any Health and Safety or Infection Control issues.
To assist in clinical practice as expected up to NVQ Level III or equivalent.
Health & Safety
Compliance with the Health & Safety at Work Act 1974 the post-holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
Mental Capacity Act 2005
The Mental Capacity Act 2005 applies to all Trust staff. It is the responsibility of every person to ensure that they keep up to date with the Act and that the delivery of patient care/services is conducted in line with local and national policy and the MCA Code of Practice.
Safeguarding is Everyones Business
The Trust has a responsibility to ensure that all children / young people and adults are adequately safeguarded and protected. As a consequence, all Trust employees are required to adhere to national and local safeguarding policies / procedures and to act upon any concerns in accordance with them.
Infection Control
It is Trust policy to reduce the risk of Healthcare Associated Infection by having in place Infection Prevention and Control Policies. These policies must be adhered to by all staff to ensure patients are cared for in a clean environment and receive the highest standards of clinical care.
Smoke Free NHS
The NHS is Smoke - Free. Smoking is not permitted on any of our premises or the surrounding land including car parking facilities.
Training
The post-holder must attend any training that is identified as mandatory to their role.
