Warrington and Halton Teaching Hospitals NHS Foundation Trust is seeking a dedicated Community Dietitian to join their small, supportive dietetic team. The ideal candidate will exhibit strong clinical, communication, and teamwork skills, working autonomously to assess, diagnose, and treat patients with various conditions. This role involves managing a diverse community caseload in home, clinic, and nursing home settings across Warrington and Halton, alongside conducting patient education sessions and contributing to student training and service development.

Key responsibilities include comprehensive patient assessments, formulating individual dietary treatment programs, maintaining accurate patient records, and collaborating with a multidisciplinary team to ensure quality patient care. The Community Dietitian will also engage in communication strategies to effectively interact with patients and their families, ensuring a cohesive approach to dietary interventions and rehabilitation.
