The Fracture Liaison Service Practitioner role at Suffolk GP Federation CIC focuses on identifying and managing individuals over 50 who have experienced fragility fractures, often indicative of osteoporosis. Key responsibilities include conducting bone health assessments, creating and evaluating care plans, providing fracture prevention advice, assessing falls risks, administering denosumab injections, and contributing to the service's development through audits and training. The position requires a proactive approach, autonomy in practice, and significant travel within the West Suffolk area, necessitating a full driving license and access to a vehicle.

The ideal candidate will be a dedicated Nurse (NHS Band 6 equivalent) or Allied Health Professional, who will receive support and training to ensure they deliver high-quality, evidence-based services. The Fracture Liaison Service aims to significantly reduce the risk of subsequent fractures among eligible patients in West Suffolk, responding to the projected increase in fracture cases across Europe.
