Band 4: £26,530 to £29,114 a year per annum (pro rata for part time)  FIXED TERM 

All patients admitted to Buckinghamshire Healthcare NHS Trust should have their smoking status and habits documented in line with the public health agenda. The post holder will assist clinical teams in recording and updating this information for newly admitted patients and in the process provide smoking cessation support to help smokers who want to stop smoking

  1. To establish and maintain communication with patients, relatives and carers, working in partnership with the multidisciplinary team to provide holistic patient care,
  2. To actively assess peoples' health, safety and wellbeing whilst delivering care in relation to tobacco and offer support and advice on how to improve health and wellbeing
  3. To assist in delivering programmes of care to meet individuals' health and well-being needs, promoting patient independence as appropriate, in partnership with the multidisciplinary team
  4. To develop own knowledge, with support, and practice whilst being aware of own levels of competence and at no times undertake tasks beyond own levels of competence
