The South Tees Hospitals NHS Foundation Trust is seeking a Band 7 Specialist Palliative Care Nurse to join their Tees Specialist Palliative Care team, working 30 hours per week. This role involves providing expert palliative care knowledge and support to patients, caregivers, and health and social care professionals in a community setting. The successful candidate will operate as an autonomous practitioner while being a part of a dynamic multidisciplinary team dedicated to delivering high-quality care. Collaboration will occur with various organisations, including hospices and care homes, as well as involvement in cross-boundary work with the North of England Strategic Clinical Network.

The Specialist Nurse will offer both direct and indirect palliative care, assisting patients and professionals according to Macmillan's four Levels of Intervention. Clinical leadership and support will be provided by the Clinical Matron, Nurse Consultant, and Cancer Lead, ensuring that the role contributes to a continually evolving service focused on achieving gold standard care in the community.
