MULTI-DISCIPLINARY
The Health Improvement Practitioner role at Livewell Southwest in Plymouth is focused on reducing falls and related injuries among older adults through the Falls Prevention Team. The position involves delivering exercise programs, specifically the Falls Management Exercise (FaME) program, and conducting multifactorial falls risk assessments. Candidates will need to engage with older adults to promote physical activity and support their adherence to preventative health initiatives, while collaborating closely with physiotherapists and other professionals within a multi-disciplinary team.

This part-time role (up to 18.75 hours per week) requires a commitment to learning and developing skills in falls prevention exercise delivery, which includes completing the Postural Stability Instructor course. Responsibilities also include outreach to hard-to-reach populations, contributing to community health initiatives, monitoring and evaluating program effectiveness, and supporting health promotion events. The position is a 2-year fixed-term contract, and applicants should be prepared to work across a seven-day service.
