Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking an enthusiastic Community Mental Health Nurse to join their multi-disciplinary team as a Care Coordinator in Southampton. This role involves managing a caseload of individuals with complex mental health and social care needs, providing tailored assessments and specific interventions for service users and their families. Newly qualified professionals are encouraged to apply, and candidates should be adaptable and willing to embrace challenges in meeting the evolving requirements of the service.

The position offers professional development opportunities within a supportive environment and emphasises collaboration with multi-agency partners. While a driving license is preferred, it is not mandatory. Work hours are from 9am to 5pm, Monday to Friday, with occasional weekend or holiday shifts, and the Trust is open to discussing flexible working arrangements during the interview process.

We are looking for an enthusiastic Community mental health nurse to join our friendly multi-disciplinary team as a Care Co-ordinator.

We are able to offer an individualised program of support and professional development to work through with the encouragement from an existing motivated, positive and experienced team. Newly qualified professionals are encouraged to apply.

You will hold a caseload and undertake the role of care coordinator for those with varied complex mental health and social care needs. You will provide assessment and specific interventions to meet the needs of service users, their carers, and families. It is expected that you will be a flexible practitioner who enjoys new challenges in meeting the developing needs of the service and our service users.

The post holder will be expected to work proactively in forging professional working relationships with multi-agencies.

Car driver is preferable but not essential. You will be working 9am -5pm Monday to Friday and occasional weekend or bank holidays. We are flexible regarding working patterns and working hours.
