Carry out joint assessments with Social Service colleagues to identify individuals who care needs meet the criteria for NHS Continuing Healthcare. Proactively manage a delegated caseload and have the ability to function autonomously. Demonstrate timely forward planning and an ability to organise self. To complete relevant specific core reviews/assessments for NHS funded individuals e.g. Deprivation of Liberty/Liberty Protection Safeguards. Deal with people with complex needs and challenging behaviour, and people approaching end stage of life. Deal with difficult family situations and dynamics which require a high level of competent effort, and the ability to remain objective and use reflective practice and team support.
