The NHS Continuing Healthcare Nurse Practitioner role at NHS Norfolk and Waveney Integrated Care Board involves clinically assessing patients for NHS Continuing Healthcare eligibility across various settings, including homes, care homes, and hospitals. The practitioner will follow the National Framework for NHS Continuing Healthcare, participate in multidisciplinary team (MDT) eligibility recommendations, and manage a delegated caseload autonomously. Responsibilities include completing new referrals within 28 days, conducting joint assessments with Social Services, and performing specific reviews for NHS-funded individuals, all while handling complex patient needs and potentially difficult family dynamics.
