Hastings & Rother Healthcare is seeking a full-time Care Coordinator for their Enhanced Care Team in Hastings. The ideal candidate will have experience in General Practice or community healthcare and a commitment to supporting vulnerable individuals, particularly those with Learning Disabilities, Frail, and Palliative care needs. The role involves coordinating care, managing safeguarding concerns, and collaborating with multidisciplinary teams to enhance outcomes for families and adults. Key responsibilities include facilitating care documentation, conducting ward rounds, and ensuring compliance with safeguarding regulations.

The position offers flexible working patterns, access to the NHS pension, five weeks of annual leave plus bank holidays, and opportunities for professional development.
