Requirement to participate in the annual flu/covid vaccination clinics which generally takes place on Saturdays.

We are looking for an enthusiastic, qualified nurse to join our Practice Nursing Team. Applicants must have a minimum of 3 years post- registration practice. We value teamwork, mutual support and a friendly working environment.

Ideally candidates will be experienced Practice Nurses who currently carry out standard treatment room duties including wound care, and cytology alongside administering childhood immunisations and travel advice and vaccinations. Additional qualifications in managing long term conditions would be an advantage, but not essential. We would also consider applications from experienced nurses who wish to move into Practice Nursing.

Excellent communication and organisational skills are essential together with an ability to thrive in a changing environment.

Practice Nurse

Hours per week 20.5 hours

Tuesday 13.00-18.30

Wednesday 13.00-18.30

Thursday 8.30-14.00

Friday 8.30-12.30

Accountable to: Business Partner/Practice Nurse Team Leader

Education Qualifications:

R.G.N First Level Nurse/Adult Branch Specific Diploma or Degree with a minimum of three years post registration experience.

Diploma or equivalent in chronic disease management.

ENB 998 or equivalent.

Job Summary:

The nurse will demonstrate a high level of clinical skills developed through experience and training. They will be responsible for providing nursing services to patients, which will include treatment, screening, advice and management of patient groups with chronic diseases within the professional competence of the post holder and in accordance with agreed protocols/patient group directions.

They will work as a self-directed practitioner supported by a higher-grade practice nurse and other health care professionals to meet the needs of the practice population.

The practitioner will accept the responsibility for providing a quality nursing service to patients in accordance with the requirements of clinical governance.

Requirement to cover nursing colleagues for annual leave and sickness absence.

Roles and Responsibilities

1.General Nursing Care

Dependent upon current experience and appropriate training, the nurse should participate in the following supported by a higher-grade nurse.

Wound Management.

Ear Irrigation

Assist with minor surgery/contraception clinic where appropriate.

Management of minor injuries, referring appropriately, if necessary.

Undertake the assessment of patient vital signs.

Undertake venepuncture and co-ordinate collection and delivery of blood and other specimens.

Maintenance of clinical area adhering to health and safety regulations and infection control policies/guidelines.

Any other procedures that the nurse has had appropriate training and education to undertake.

2. Health Promotion and Disease Prevention

Dependent upon current experience and appropriate training, the nurse should participate in the following:

Cervical Cytology, breast awareness.

Awareness of Testicular Self-Examination.

Immunisation and vaccinations in line with agreed protocols.

Comprehensive Travel health advice.

Opportunistic coding and updating patient records in line with Quality and Outcomes Framework (QoF)

Participate in health promotion activities to meet local and national guidelines. (HIMP and NSF etc)

Assist in screening procedures to agreed protocols.

Advise patients opportunistically on general health care and healthy lifestyles

3. Disease Management:

Dependent upon current experience and appropriate training, the nurse should manage and run Nurse led clinics or assist in the management of GP led clinics within agreed practice protocols, for example: -

- Asthma

- Diabetes.

- Coronary Heart Disease Management.

- Hypertension.

- Warfarin Management.

To work towards the attainment of appropriate qualifications, in order to maintain a high level of knowledge in disease management.

Advise patients on the use of equipment to manage their condition.

4. Administration:

Dependent upon current experience and appropriate training, the nurse should participate in the following supported by a higher-grade nurse and practice administration staff.

Maintain accurate patient records.

In conjunction with practice staff, undertake clinical audit ensuring that relevant patient health data is collected.

Assist in the maintenance of Disease registers and assist in the call and recall of patients.

Liaise with GP/Senior nurse/Business Partner to order equipment and supplies maintaining adequate stocks of items such as dressings, drugs and instruments, specimen bottles, diagnostic agents and utensils (ensuring that apparatus is properly sterilized).

Assist in maintaining the treatment room in a clean and tidy manner.

To participate as a team member in the formulation of practice philosophy, strategy and policy.

5. Professional Development:

Continually update professional skills and knowledge, undertaking competencies congruent with grade.

Maintain a Professional Portfolio.

Willing to attain further professional qualifications.

6. Educational:

Manage, supervise, teach and support newly appointed staff.

Maintain a high standard of research based nursing expertise.

Contribute to the continuous development of policies and protocols ensuring that evidence based practice is maintained.

The post holder must be in possession of a current Nursing and Midwifery Council pin number and maintain current membership of a professional organisation, e.g.: RCN, MDU.
