Candidate: Support Time Recovery Worker (STR)
An exciting opportunity has arisen for a band 4 STR to join the Fylde and Wyre Community Prevention and Engagement Team (CPET) in Mental Health Rehabilitation services.
LSCFT Specialist Mental Health Rehabilitation is currently undergoing transformation across inpatient and community services in order to implement a robust pathway for people with complex mental health needs in the least restrictive and safest way, there is a need to develop the range of service provision that will enable this. The aim of the transformation is to provide a service which can offer a whole pathway of support locally, identified in both national plans and clinical guidance, which would include an inpatient and community service offer that is matched against the identified need of LSCFT population.
In order to achieve our aims LSCFT MH Rehab needs to standardise the approach to the provision of these services; to identify and reduce inequalities and improve quality, ensure services are safe, effective, evidence based and informed. The MH Transformation programme of work ensures we are working towards an end to end service pathway that will support people with the most complex mental health needs at the right time and in the right place. Whilst the current role is assigned to CPET this role may change in the future, this might mean working with people with greater mental health complexities.
An exciting opportunity has arisen for a highly skilled practitioner to join the Community Prevention and Engagement Team. The ideal candidate will have experience, specialist interest in rehabilitation, and expertise in mental health who will be working with an array of mental health services and key stakeholders. This team holds the importance of community rehabilitation and social inclusion at its heart. Working from the philosophy that people achieve a more sustained recovery when community connection is apparent.
The post holder will work as part of a team that provides Community support in Mental Health Services, to positively promote recovery, life skills and social inclusion by focusing on the direct needs of service users and/or carers, working across all mental health pathways.
