The Mid and South Essex NHS Foundation Trust is seeking a Senior Hospital at Home Nurse for their Hospital@Home service based at Broomfield Hospital in Chelmsford. This role involves providing professional and clinical leadership while delivering care to sub-acutely unwell patients in their homes. Key responsibilities include administering intravenous medications, wound care management, supporting admission avoidance, and managing patient assessments in both home and clinic settings. The successful candidate will also oversee care plans, address patient needs, and may act as a deputy for the virtual ward manager when necessary.

This position requires a registered nurse with a full driving license and access to a vehicle for community-based work. The nurse will work within the senior nursing team to support the effective operation of the service across multiple hospital sites, ensuring the delivery of holistic, one-on-one patient care while facilitating patient flow and discharge planning. Travel expenses for work-related travel will be reimbursed.
