The Calderdale and Huddersfield NHS Foundation Trust is seeking a dedicated Heart Failure Specialist Nurse to join their integrated Heart Failure Service in Sowerby Bridge. The role involves developing and managing a comprehensive care program for patients with heart failure across various settings, including acute, community, and virtual wards.
