The Somerset NHS Foundation Trust is seeking a Mental Health Practitioner or community mental health nurse with a keen interest in Dual Diagnosis for its Mendip Neighbourhood Older Persons Mental Health Service. This part-time role involves providing assessment and treatment for individuals over 65 with severe and enduring mental health issues, including those with coexisting substance misuse disorders. The successful candidate will act as a key worker, developing and reviewing care plans, coordinating treatment, and offering direct interventions, along with supporting and advising other staff.

The position can be based at either Chantry Day Hospital in Frome or Willow Bank, Priory Health Park, Wells, with flexible working arrangements available. The role emphasises collaboration with multidisciplinary teams to promote best practices in the assessment and treatment of service users, and includes ongoing opportunities for professional development. Applicants will have the chance to enhance their clinical skills and support health education initiatives within a supportive environment.
