The Torquay Health and Well-Being Team is looking for a Band 6 Occupational Therapist to conduct holistic assessments and manage a complex caseload in community settings. The role involves working autonomously with service users, their families, and other health care professionals to provide tailored interventions aimed at maintaining patients in their homes and preventing hospital admissions. Candidates will be responsible for assessing and implementing care plans, ensuring safe equipment provision, and contributing to discharge planning from temporary care facilities.

The position requires flexibility to cover a seven-day service, including weekends and bank holidays. Applicants should possess a Diploma or Degree in Occupational Therapy, valid HCPC registration, and proven post-graduate experience. Strong decision-making capabilities under pressure, effective communication skills, and experience in supervising junior staff or students are also essential. The role emphasises a collaborative approach with general practice and health and social services to fulfill the Care Trust's integration goals.
