Symphony Healthcare Services is seeking a Practice Nurse to join their team at Lister House Surgery in Wiveliscombe, Somerset, to cover parental leave. This part-time position, requiring 27 hours per week for a fixed term of 7-8 months, involves providing optimal care to the practice population, emphasizing health prevention, monitoring long-term conditions, and engaging in screening activities. The nurse will collaborate with the general practice team to ensure patient needs are met while supporting policy implementation and offering nurse leadership as necessary.

Symphony Healthcare Services (SHS) are looking to recruit a Practice Nurse to cover parental leave at their Lister House Surgery in Wiveliscombe, Somerset.
As a SHS Practice Nurse you will:
- be part of a nursing team responsible for ensuring that the practice provides optimum care relevant to the practice population, either in practice or the home environment.
- focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives and vision of Symphony Healthcare Services.
- work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
This is a part time post of 27 hours per week for a 7/8 month fixed term contract, covering parental leave.
The annual salary range for this post is £39,146 - £41,851 per annum (pro rata for part time hours).

The main duties of a SHS Practice Nurse are:
- Assisting the whole practice team to help deliver effective implementation of management of patients and their needs.
- Managing the nursing care of patients with chronic disease, employing recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract purposes is also required
- Contributing to the development of policies / protocols / guidelines within the practice.
- Aiding the practice team with implementation of new initiatives in the delivery of chronic disease management services.
- Encouraging the development of integrated services within Primary Care to include integrated practice based teams and multidisciplinary working.
