The role of a Rehab Support Worker at Frimley Health NHS Foundation Trust in Fleet involves providing hands-on care and support to patients in their homes, aimed at promoting independence and assisting with their rehabilitation, either to prevent hospital admissions or aid in discharge. The position requires working as part of a multidisciplinary team and includes responsibilities such as delivering personal care, applying rehabilitation techniques, and supporting end-of-life patients and their families.

Successful candidates should possess strong communication skills and be confident in lone working, as well as being able to travel independently between patient homes. The role entails a commitment to continuous professional development and requires working shifts of 37.5 hours a week between 8 am and 9 pm. Applicants must have the right to work in the UK and hold valid business insurance for their vehicle.
