The Community Mental Health Recovery Practitioner role within the New Community Enablement and Recovery Team (CERT) at Black Country Healthcare NHS Foundation Trust involves providing comprehensive care coordination and support for individuals with severe mental illness. The practitioner will work with a capped caseload of service users, focusing on those transitioning from out-of-area rehabilitation placements or residing within the Black Country, ensuring that their complex mental health needs are met through tailored recovery-focused interventions. The position emphasises building resilience, improving wellbeing, and facilitating a step-down to local services while collaborating closely with a multidisciplinary team, including psychiatrists, psychologists, and occupational therapists.

Additionally, the practitioner will support hospital discharges and liaise with external agencies to promote personalized recovery outcomes. Flexibility is key, as the role encompasses community-based work, and some travel will be required. The service operates Monday to Friday, with a commitment to guiding service users on accessing support outside of standard hours. The position contributes to the overall development of the CERT while prioritizing holistic approaches to enhance the recovery journeys of patients.
