The role of Senior Allied Health Professional at Imperial College Healthcare NHS Trust focuses on leading a 12-month fixed-term project within the North West London Inner Boroughs Pulmonary Rehabilitation Service. This position entails addressing service inequity and promoting long-term self-management for patients with respiratory issues. The practitioner will work collaboratively with a Band 6 Social Prescriber and a Band 5 Exercise Practitioner to identify areas of low referral and uptake, particularly in underprivileged communities, and enhance service awareness in hard-to-reach areas. Responsibilities include conducting post-rehabilitation follow-up clinics, providing high-quality care, supporting patients and families, and ensuring smooth transitions of care within the community.
