The Specialist Clinical Pharmacist role at Mid Yorkshire Teaching NHS Trust is an exciting opportunity for a proactive and dedicated pharmacist to join the Care of the Elderly pharmacy team. This position involves ensuring safe and effective prescribing and administration of medications, particularly during patient discharge. The pharmacist will primarily be based at Pinderfields Hospital but may need to work across the Trust’s three hospital sites. Responsibilities include prescribing discharge medications, writing discharge summaries, collaborating with a multidisciplinary team to facilitate safe patient transitions, and contributing to the implementation of Medicines Optimisation strategies.

Candidates should possess a post-graduate diploma in clinical pharmacy or equivalent experience, be a non-medical prescriber or currently in training for this qualification, and must be registered with the General Pharmaceutical Council. Strong communication skills, experience in a clinical setting, and a commitment to ongoing professional development are essential. Additional responsibilities may include participating in service audits, risk management, and supporting various clinical services across the Trust. Flexibility to cover for colleagues during absences is also required.
