The Royal Devon University Healthcare NHS Foundation Trust is seeking a Medicines Management Technician for the Acute Hospital at Home (AHAH) service in Exeter. This role involves providing a safe and cost-effective pharmacy service, offering medicines optimisation advice to colleagues, patients, and stakeholders, and facilitating the safe transfer and discharge of patients concerning their medications. The successful candidate will collaborate with a multidisciplinary AHAH team, which includes pharmacists, doctors, and nurses, to ensure the appropriate management and reconciliation of patient medications while communicating effectively with all parties involved in patient care.

Candidates should have a strong background in hospital pharmacy, particularly in medicines reconciliation and discharge processes, along with excellent communication skills. The position requires flexibility and the ability to work independently within a fast-paced environment that prioritizes patient care. The role is full-time, involving participation in a 7-day working pattern, and supporting patient services across Northern and Eastern areas.
