The Clinical Pharmacist role at South Bucks Primary Care Network offers an exciting opportunity for an enthusiastic candidate to join the team at Threeways Surgery in Stoke Poges, with a salary of up to £44,500 WTE, based on experience. This full-time position (37.5 hours per week, part-time options available) involves a patient-facing role where the pharmacist will utilize their clinical skills to manage long-term conditions, support hospital discharge prescriptions, and conduct medication reviews for patients on multiple medications. The post holder will be supported in their professional development, including opportunities to complete the CPPE Primary Care Pharmacy Education Pathway and working towards independent prescribing qualifications.

Key responsibilities include collaborating with a multidisciplinary team, managing chronic disease areas, and developing medicines management plans to improve the quality of patient care. The Clinical Pharmacist will also play a central role in shared care protocols, conduct clinical research related to medications, and build relationships with other healthcare professionals to address public health and social care needs within the community.
