The role of the Team Leader is to provide high quality care and support, as defined in the care and support plan, to our clients within the clients' home, therapeutic environment or in the community. The Team Leader is responsible for managing the support team to ensure day to day care and strategic outcomes are met.
The Team Leader is expected to provide leadership and support for care staff, act as a role model for good clinical and care governance practice, uphold the rights, choice and dignity of Service Users, and ensure Libertatem Healthcare Group policies and procedures are followed at all times. They are also responsible for managing the team, including rota management, supervision, feedback, and ongoing training needs, as well as delivering a high-quality care service without discrimination.

The Support worker must at all times respect the confidentiality of information in line with the requirements of the Data Protection Act. This includes, if required to do so, obtain, process and/or use information in a fair and lawful way, and to disclose data or information only to authorised persons or organisations as instructed.
TrainingThe Team leader will complete the mandatory and client specific training both in- house and external to achieve the specified level of competency for each client with whom they work.
Prepare for, attend and participate in regular supervision and appraisal meetings with the healthcare coordinators and clinical lead nurses.
Demonstrate a commitment to personal development.
Team leader will be proactive in providing suggestions to clinical lead on any additional training that may benefit the team in delivering bespoke and client centered care and support
