The Central and North West London NHS Foundation Trust is seeking a dedicated Community Mental Health Practitioner / Psychiatric Triage Nurse for its Harrow Community Mental Health Triage and Assessment Team. This role involves providing comprehensive psycho-social assessments to establish the care needs of adults with mental health issues, and the successful candidate will manage a defined caseload while liaising between primary and secondary care providers.
