The Hampshire and Isle of Wight Healthcare NHS Foundation Trust is looking for an experienced Band 6 Older Persons Mental Health Practitioner to join their Mental Health Liaison Team in Cosham. They also welcome applications from Band 5 practitioners who are keen to advance to a Band 6 role within 12-18 months. Ideal candidates should be innovative, motivated, and compassionate mental health professionals eager to develop their skills through comprehensive training, including weekly complex case discussions and daily multidisciplinary team (MDT) meetings. The role offers a dynamic opportunity to work with adult patients, particularly those over 18 with acute mental health issues or exacerbations of existing conditions, while collaborating closely with physical health colleagues at Queen Alexandra Hospital.

Key responsibilities include conducting holistic biopsychosocial mental health assessments, administering personalized interventions, creating recovery-focused care plans, and providing tailored support. The Trust also supports flexible working arrangements and offers a preceptorship program for newly qualified applicants.
