In this role, you will work within a GP practice, providing patient-facing care such as triage, clinical assessments, and home visits alongside a diverse team of healthcare professionals. Your responsibilities will include diagnosing and managing both acute and chronic conditions, performing specialist assessments, and making referrals as necessary, while also delivering high-quality, person-centered care to the local community.
